Job Description:
A company is looking for a Health Advocate to support patients
with chronic kidney disease and end-stage kidney disease.
Key Responsibilities
Coordinate care with the clinical team and assist members with
healthcare appointment scheduling and resource location
Conduct outreach to inform members about the Kidney Services
program and help them access community and government-based
services
Build rapport with members through communication and provide
excellent customer service while documenting interactions
accurately
Required Qualifications
High School Diploma / GED
Must be 18 years of age or older
2+ years of prior medical/healthcare experience in relevant
fields
Proficiency in Microsoft Word, Excel, and Outlook
Ability to work 40 hours per week, Monday - Friday, with potential
flex hours
