the work that you do.Job OverviewThis is a grant funded position
through SAMHSA contract (H79SM086855) funded from 09/30/2022-
09/30/2026. This position will be attached to the Pomona Outpatient
Behavioral Health site. The client population consists of pregnant
and parenting (perinatal) women, criminal justice involved,
mandated and community clientele from different walks of life. The
goal of the Certified Community Behavioral Health Center (CCBHC)
Program is to integrate mental health and substance use disorder
treatment (behavioral health) and physical health allowing clients
to receive whole-person, streamlined system of care targeting their
primary and behavioral healthcare needs. This position will work as
part of a treatment team with a targeted approach to quantify
treatment outcomes through periodic interviews with clients to
complete the National Outcome Measure Surveys required as part of
the CCBHC grant funding requirements.The primary responsibilities
of this role are focused around client interactions through
interviews at the beginning of a client's treatment episode, at a
mid-point of their treatment episode and at the end of their
treatment episode in order to identify change to behavioral health
symptomatology, effective treatment services and overall program
outcomes. Consistent, efficient and accurate documentation is
required for this role. Experience with software systems and
Electronic Health Record (EHR) navigation is highly preferred. The
ability to manage time, work within deadlines and communicate both
verbally and in writing are required. Clinical interviewing skills,
with respect to trauma experiences and cultural competence is
imperative to the Data Coordinator role.Key Responsibilities---
Engages with clients at distinct intervals (intake, 6-month mark,
discharge) in their treatment episodes to complete clinically and
culturally appropriate interviews.--- Documents interview (and
non-interview, when interviews are not appropriate) results in the
National Outcome Measure Survey (NOMS) measure in the client's EHR
record within 2 business days.--- Ensures timely and appropriate
billing documentation as required.--- Assists with linkage and
referrals for CCBHC services, as applicable.--- Seeks clinical
consultation when indicated.--- Works collaboratively with other
members of the treatment team including the Therapist, Counselor,
Medical, and Psychiatrist.--- Delivers services in outpatient,
residential, or field depending on assignments and client
need.Education and Knowledge, Skills and AbilitiesEducation
Requirements:--- Substance Abuse Counselor (SUD) Certification:
from an approved/accredited California agency.--- Bachelor's Degree
from an accredited university.Experience:--- Preferably a minimum
of two years' experience in the human service field and
demonstrated expertise in substance abuse treatment, relapse
prevention, and recovery.--- Experience working with clients
experiencing acute withdrawal from substances.--- Knowledge of and
experience with providing trauma informed services.--- Experience
delivering evidence-based practices.Skills:--- Professionalism,
punctuality, flexibility and reliability are imperative.---
Excellent verbal, written, and interpersonal skills.--- Bilingual
in Spanish.In compliance with the California Department of Public
